ECPR for prolonged Pediatric Cardiac Arrest, an outcome without major neurological compromise.

چکیده

Pediatric in-hospital cardiac arrest (IHCA) has been reported in 1 – 3% of pediatric intensive care unit (ICU) admissions and up to 6% children ICU. In the last 25 years, survival hospital discharge after IHCA improved from 9% 13.7% 35%. The improvement outcomes was attributed part apparition ECMO as a rescue strategy when prolonged conventional CPR cannot restore spontaneous circulation. We report case 4th-month-old patient with history ventricular septal defects, left right shunt enlargement heart chambers that underwent surgery for closure atrial experienced complications led use response arrest.

Neurological outcome of prolonged coma survivors of out-of-hospital cardiac arrest.

Nine adult survivors of out-of-hospital presumed cardiogenic cardiac arrest, who remained unresponsive for longer than their third hospital day and who eventually were discharged from the acute care hospital, are reported. Their neurological outcome fell into three distinct categories: (a) a persistent vegetative state, (b) able to follow some simple commands but requiring total nursing care, a...

Oximetry-guided reoxygenation improves neurological outcome after experimental cardiac arrest.

BACKGROUND AND PURPOSE Current guidelines suggest that cardiac arrest (CA) survivors should be ventilated with 100% O(2) after resuscitation. Breathing 100% O(2) may worsen neurological outcome after experimental CA. This study tested the hypothesis that graded reoxygenation, with oximetry guidance, can safely reduce FiO(2) after resuscitation, avoiding hypoxia while promoting neurological reco...

Therapeutic hypothermia and neurological outcome after cardiac arrest.

INTRODUCTION/AIM The most important clinically relevant cause of global cerebral ischemia is cardiac arrest. Clinical studies showed a marked neuroprotective effect of mild hypothermia in resuscitation. The aim of this study was to evaluate the impact of mild hypothermia on neurological outcome and survival of the patients in coma, after cardiac arrest and return of spontaneous circulation. M...

Serum tau and neurological outcome in cardiac arrest

OBJECTIVE To test serum tau as a predictor of neurological outcome after cardiac arrest. METHODS We measured the neuronal protein tau in serum at 24, 48, and 72 hours after cardiac arrest in 689 patients in the prospective international Target Temperature Management trial. The main outcome was poor neurological outcome, defined as Cerebral Performance Categories 3-5 at 6 months. RESULTS Inc...

Sodium bicarbonate improves outcome in prolonged pre-hospital cardiac arrest.

OBJECTIVE This study evaluates the effect of early administration of an empirical (1 mEq/kg) sodium bicarbonate dose on survival from prehospital cardiac arrest within brief (<5 minutes), moderate (5-15 minutes), and prolonged (>15 minutes) down time. METHODS Prospective randomized, double-blinded clinical intervention trial that enrolled 874 prehospital cardiopulmonary arrest patients manage...
